Life Events
As your life changes, so do your benefit needs. Your UNFI benefit elections stay the same all year unless you have a change in employment or family status and make a new election.
Overview
You cannot change your elections for medical, dental, vision and FSA coverage during the year unless you experience a Qualifying Life Event, including:
- Marriage
- Divorce/legal separation
- Birth/adoption of a child
- Death of spouse or dependent
- Spouse becomes eligible for other coverage (such as through his or her employer)
- Other Qualifying Live Events as noted in the UNFI Health and Welfare Plan Wrap Summary Plan Description (Wrap SPD)
Initiate a Life Event
If you experience a Qualifying Life Event, you must notify and provide any required documentation to UNFI within 31 days of the event (including the date of the event). To initiate a qualifying life event online:
- Prior to January 1, 2027: Go to the HR Self-Service Portal.
- January 1, 2027 and after: Go to UNFIBenefits.com.
If you lose or gain coverage for a state premium assistance program under Medicaid or Children’s Health Insurance Program (CHIP), you have 60 days from the date of the eligibility change to request enrollment or cancellation of your benefits.
Benefit changes must be consistent with the life event. For events relating to the loss or gain of other coverage, the event date is the date the other coverage was terminated or the effective date of the other coverage, as applicable.
Contact the UNFI Benefits Support Center.
